I run bmitool.org. It’s a small site — maybe 5,000 visitors a month, mostly people googling “BMI calculator” and stumbling onto my version. But I get about 20 emails a month from people who are confused, frustrated, or both. The most common subject line: “I lost weight but my BMI barely changed” or the more desperate version: “I lost 40 lbs and my body looks completely different but my scale and my BMI say I’m still obese? What am I doing wrong?”

Nothing. You’re doing nothing wrong. You’re just running into the limitations of BMI as a metric for individuals who have changed their body composition.

Let me use a real email (shared with permission, anonymized). “Hey Jamie, I’m 5‘6” female, 42 years old. Started at 210 lbs (BMI 33.9, obese class 1). Got down to 170 lbs (BMI 27.4, still overweight). I’ve lost 40 pounds. I wear a size 8 now (down from size 16). I ran a 5k last month for the first time in my life. But my doctor looked at my chart and said ‘you’re still overweight, keep working on it.’ I feel like giving up.”

I built a simulation to answer her question. Same height (66 inches), different weights, plus a third variable: estimated lean mass (muscle). The simulation uses standard body fat prediction equations from the National Academy of Sports Medicine.

Her reported size 8 corresponds roughly to the “high muscle” scenario at 170 lbs — about 29% body fat. For a 42-year-old woman, the American Council on Exercise (ACE) classifies 25-31% body fat as “fitness” range and 32%+ as “overfat.” She’s in the fitness range at 29%. But her BMI says overweight (27.4). Why the disconnect? Because BMI doesn’t know she built muscle during the loss. She told me she started strength training (body pump classes at Life Time) at 190 lbs and kept it up. She gained about 3-4 lbs of lean mass while losing 40 lbs total.

A 2022 study in the *European Journal of Clinical Nutrition* followed 150 women who lost 30+ lbs through a combination of diet and strength training. At their final weight, average BMI was 26.8 (overweight by standard cutoffs), but average body fat was 28% (healthy fitness range for women). The disconnect between BMI and actual health outcomes is largest in people who exercise while losing weight — which is exactly the population who should feel *good* about their progress, not discouraged.

So what should this person do? Ignore BMI entirely? No. But add another metric. I told her: measure your waist. At 170 lbs, if your waist is under 33 inches (height in inches * 0.5 = 33 inches), you’re metabolically fine regardless of BMI. She measured: 31.5 inches. Waist-to-height ratio: 31.5 / 66 = 0.48 (well below the 0.5 risk cutoff). I sent her a link to a 2023 *JAMA* study that showed WHtR < 0.5 is associated with low cardiometabolic risk even in people with BMI > 25. She forwarded that to her doctor. The doctor wrote back: “I didn’t know that. Thank you. You’re doing great. Ignore the BMI.”

Here’s the practical takeaway for anyone reading this who has lost significant weight (20+ lbs) and is confused by their BMI: Compare your current waist measurement to your height. If waist/height < 0.5, you’re likely in a healthy risk category even if BMI says overweight. If waist/height is between 0.5 and 0.6, you have moderate risk — focus on reducing waist, not necessarily weight. If waist/height > 0.6, high risk — keep going. The woman from the email? She’s now maintaining at 168-172 lbs, size 8, running 10ks, and doing strength training twice a week. Her BMI still says 27-27.5 (overweight). She doesn’t care anymore. And she shouldn’t.

One more note: If you’re losing weight and not strength training, about 20-30% of the weight you lose will be lean mass (muscle, bone density, water). That’s the normal ratio for diet-only weight loss. But if you’re strength training while losing weight, you can reduce that to 10-15% or even zero (recomposition). The woman in the email preserved her muscle. That’s why her BMI didn’t drop as much as expected — she lost fat but kept muscle, so weight loss was slower but healthier. This is a feature, not a bug. Tell your doctor. Bring the WHtR data. And keep lifting.

I see this pattern in the emails I get from Austin folks more than anywhere else. Maybe it's the CrossFit culture, maybe it's the running trails around Lady Bird Lake, but Austinites love cardio and hate the weight room. I get it — I was the same way. But the data doesn't lie: if you're losing weight and your BMI is stuck, check your waist and your lifting log. One of those numbers is moving. Track the right one.
